13. 2005/06: Swansea City
After a cruel penalty shoot-out defeat to Barnsley in 2006’s final, the Swans came mighty close to another shot at promotion the following season - but finished just short. Finished the year after play-offs: 7th. Photo: Catherine Ivill
14. 2004/05: Hartlepool United
In one of the most dramatic play-off finals of recent years, Pools were undone in extra time by a strong Sheffield Wednesday side. They couldn’t go on to repeat their heroics. Finished the year after play-offs: 21st (relegated). Photo: Paul Thomas
15. 2003/04: Bristol City
The Robins lost to Brighton in 2004’s play-off final and, similarly to Swansea City before them, couldn’t quite reach the top six again. Finished the year after play-offs: 7th. Photo: Harry Trump
16. 2002/03: QPR
It’s easy to forget that QPR spent time in the third tier, and they were denied promotion in 2003 by an impressive Cardiff City side - before bouncing back in the best way. Finished the year after play-offs: 2nd (promoted). Photo: Henry Browne
