Nathan Broadhead scored the only goal of the game in the 10th minute after he was sent through on goal by Corry Evans.
Broadhead was then forced off with an injury before half-time and was replaced by Jack Clarke.
Sunderland will now face Sheffield Wednesday in the play-offs.

What to make of that side
So it’s three changes from the Sunderland side which started against Rotherham on Tuesday.
Alex Pritchard, Ellot Embleton and Patrick Roberts have all returned to the side in place of Callum Doyle, Trai Hume and Jay Matete.
Doyle hasn’t even been named on the bench, yet Aiden McGeady has returned to the matchday squad for the first time since November.
It’s unclear if Sunderland will start with a back three or back four with one less centre-back in the squad.
Dennis Cirkin and Carl Winchester remain sidelined through injury.
