A CORONER slammed tragic care worker Ashleigh Ewing’s boss for failing to take steps to prevent her death.
David Mitford accused Sunderland-based Mental Health Matters project manager Steve Brown of “passing the buck” after it was revealed a risk assessment of her mentally ill killer, Ronald Dixon, was three years out of date.
He made the comments during an inquest into Miss Ewing’s death, in May 2006.
Mr Mitford said Miss Ewing, who was stabbed to death by Dixon, had only the 2003 assessment to work from. The inquest continues.
