PATIENTS are being urged to share their experience of being discharged from hospital.
NHS County Durham is running a survey to obtain feedback on the process.
Views are sought from patients or the carers of patients who have been discharged from hospital in the last 12 months.
Head of operations Matthew James, said: “Discharge planning is one of a number of priorities for the NHS in the North East, and we’re already working with service user and carer interest groups and patient networks across the county to drive this forward.
“The results of the survey and discussion groups will outline exactly where improvements need to be made,”
